Specimen Types and Testing Options

Available specimen types and testing methods vary by service, purpose, and laboratory requirements.

Urine Drug Testing

DOT Available

Urine drug testing is the standard specimen type for DOT-regulated testing and is widely used for Non-DOT employment, court, probation, parole, recovery, and personal testing. Available as laboratory-based testing or rapid point-of-care screening for approved Non-DOT purposes.

Laboratory-based urine drug testing
Rapid point-of-care urine screening for approved Non-DOT purposes

Hair Drug Testing

Non-DOT

Hair drug testing provides an extended detection window and may be used for Non-DOT employment, court, probation, parole, recovery, and other approved purposes. Hair testing is not a DOT-approved specimen type for federally regulated testing.

Hair drug testing for approved Non-DOT purposes
Extended detection window
Laboratory-based analysis

Hair drug testing is a Non-DOT service and should not be presented as part of federally regulated DOT drug testing.

Oral-Fluid Drug Testing

Non-DOT

Current federal rules authorize oral fluid as a specimen type. However, DOT-regulated oral-fluid testing cannot be implemented until at least two HHS-certified oral-fluid laboratories are available and the applicable federal operational requirements are satisfied. Non-DOT oral-fluid testing may be available separately when an appropriate laboratory, approved device, collection process, and service workflow are in place.

Non-DOT oral-fluid testing — Availability Pending
DOT oral-fluid testing — Not yet implemented (pending HHS laboratory certification requirements)

DOT oral-fluid testing is not currently available. Non-DOT oral-fluid testing availability will be confirmed at the time of service.

Sweat Patch Drug Testing

Non-DOT

A patch worn on the skin that continuously collects perspiration over several days or weeks. Sweat patch testing is commonly used for court supervision, probation, parole, sober-living programs, recovery monitoring, reentry programs, and other approved Non-DOT monitoring purposes. Sweat patch testing is not a DOT-approved specimen type.

Court-ordered sweat patch testing
Probation and parole monitoring
Sober-living and recovery program monitoring
Reentry program monitoring
Attorney-requested testing
Other approved Non-DOT monitoring

Sweat patch drug testing is a Non-DOT service and should not be presented as part of federally regulated DOT drug testing.

Nicotine Testing

Non-DOT

A Nurse's Journey provides nicotine and cotinine testing for approved Non-DOT purposes. Cotinine is a substance produced by the body after exposure to nicotine and may remain detectable longer than nicotine itself.

Nicotine or cotinine testing may be requested for:

Employer wellness or tobacco-use programs
Workplace policy requirements
Insurance or wellness-program documentation
Healthcare or surgical-program requirements
School or training-program requirements
Personal testing
Other approved Non-DOT purposes

Available specimen options may include:

Urine nicotine or cotinine testing
Oral-fluid nicotine or cotinine testing, when available for Non-DOT purposes
Hair nicotine testing, when available
Laboratory-based testing
Rapid screening options, when available

Nicotine testing may detect exposure associated with cigarettes, cigars, smokeless tobacco, vaping products, nicotine pouches, nicotine gum, nicotine patches, and other nicotine-containing products. Testing may not determine the exact nicotine product used, the precise time of exposure, the amount used, or whether exposure was intentional. Nicotine testing is a Non-DOT service and should not be presented as part of federally regulated DOT drug testing.

Testing Programs and Reasons for Testing

A Nurse's Journey supports a range of testing programs for employers, courts, attorneys, recovery programs, schools, and individuals.

Random Drug Testing

Random drug testing may be used by employers and approved organizations as part of a written testing program. Selections should be made using a neutral and appropriate random-selection process that gives covered participants an equal chance of being selected during each testing period.

Testing may be available for:

DOT-regulated employers
Non-DOT employers
Recovery and monitoring programs
Sober-living facilities
Probation or parole programs
Other approved organizations

DOT random testing must follow the requirements of the applicable federal transportation agency.

Reasonable-Suspicion Drug Testing

Reasonable-suspicion or reasonable-cause testing may be requested when a trained employer representative or another authorized party documents specific observations that meet the applicable policy or regulatory requirements.

Testing may be available for:

DOT-regulated employers
Non-DOT employers
Schools or training programs
Recovery or treatment programs
Other approved organizations

A Nurse's Journey performs the requested collection or testing service but does not determine whether reasonable suspicion exists. The employer, agency, or requesting organization is responsible for making and documenting that determination.

Post-Accident Drug Testing

Post-accident or post-incident testing may be requested following a workplace, transportation, safety, or organizational incident. Testing requirements may depend on:

Testing requirements may depend on:

DOT agency regulations
Employer policy
Type of incident
Timing requirements
Employee role
Injury or property-damage criteria
Required drug or alcohol test
Collection-site availability

A Nurse's Journey does not determine whether an incident meets DOT or employer post-accident testing criteria. The employer or authorized requesting party must make that determination.

Return-to-Duty Drug Testing

Return-to-duty testing may be required before an employee returns to safety-sensitive or other covered duties following a policy or regulatory violation. DOT return-to-duty collections must follow applicable federal requirements, including direct-observation procedures when required. Return-to-duty testing may also be used under an approved Non-DOT employer, court, treatment, recovery, or program policy.

A Nurse's Journey does not independently authorize an employee or participant to return to work, duty, housing, court supervision, or program participation.

Follow-Up Drug Testing

Follow-up testing may be required after a return-to-duty process, treatment plan, court order, probation or parole requirement, recovery plan, or other approved monitoring arrangement. Testing frequency, duration, specimen type, testing panel, and observation requirements must be established by the authorized employer, Substance Abuse Professional, court, agency, attorney, probation or parole program, treatment provider, or other requesting party.

A Nurse's Journey provides the approved collection and testing coordination but does not create the follow-up testing plan unless specifically authorized and qualified to do so.
